What is the 1095-C form?


The Affordable Care Act (ACA) requires AT&T to provide a Form 1095-C to all employees, former employees (Retirees) and dependents with health coverage* through AT&T. AT&T will send you a completed Form 1095-C in late January. You will need the information to complete your 2015 tax return. This is proof of health coverage which the ACA requires you to have or you will have to pay a tax penalty.  For your reference, a sample Form 1095-C  is included. The 1095-C includes information on medical coverage received through AT&T during any portion of 2015. The AT&T Form 1095-C  does not include information about medical coverage received through a third party,  such as another employer or an insurer. * Health coverage includes Medical, CarePlus or a Health Reimbursement Account (HRA).
