Advocate Health Care has notified UnitedHealthCare, that as of January 1, 2004, its hospitals and medical groups have chosen to no longer participate in the UnitedHealthCare Network. Advocate Health care or medical group physicians, services provided on or after January 1, 2004, will no longer be covered according to your in-network benefits.
